A single center 14 years study of infectious complications leading to hospitalization of patients with primary antibody deficiencies BJID

Primary antibody deficiencies (PADs) are a heterogeneous group of disorders, characterized by hypogammaglobulinemia and increased susceptibility to bacterial infections, leading to hospitalizations. This study was performed to determine the main infectious causes of hospital admissions in selective Iranian patients with PADs. Forty patients with PADs, who were admitted to the Infectious Ward of Children's Medical Center Hospital during a 14-year period, were reviewed in this study. There were 115 documented episodes of hospital admission during a 14-year period. The average length of hospital stay was 33.30 ± 25.72 days. BCGiosis as a presenting feature of a child with chronic granulomatous disease BJID

Bacillus Calmette Guerin (BCG) vaccine, which is administered to all newborns in some regions, could lead to serious complication ranging from local disease (known as BCGitis) to disseminated disease (BCGosis) in a group of patients with primary immunodeficiency diseases. We are reporting here a 3.5 year-old girl with a history of prolonged BCGitis, which developed to disseminated disease without any other special features. Immunological studies with nitro-blue tetrazolium test confirmed the diagnosis of chronic granulomatous disease in this patient. Chronic granulomatous disease should be considered in the list of differential diagnosis in all children with BCGosis, even in the absence of any other manifestations related to immunodeficiency.

Candida albicans meningitis in an infant with noonan syndrome BJID

Noonan syndrome is a rare disorder, characterized by several malformations such as dysplasia and stenosis of the pulmonary valve, atrial septal defect and a typical pattern of hypertrophic cardiomyopathy. We describe here a 1-month old girl, who was referred to our center with seizure and apnea. She had wide anterior fontanel, head circumference and sunset eye. Intaventricular hemorrhage by sonography and atrial septal defect and hypertrophy cardiomyopathy by echocardiography were detected. Clinical and laboratory findings of the patient were compatible with a diagnosis of Noonan syndrome, which was also confirmed by molecular analysis. Cutaneous cytomegalovirus infection in a child with hyper IgE and specific defects in antibody response to protein vaccines BJID

Cytomegalovirus (CMV) infection is a common opportunistic systemic infection in immunocompromised patients, but skin involvement is rare. Herein, we report a 10 year-old girl from consanguineous parents who was referred to our center because of disseminated maculopapular rash. She had history of upper and lower respiratory tract infections. In immunological studies, increased serum IgE level and decreased responses to tetanus and diphtheria were detected. Polymerase chain reaction (PCR) examination of bronchoalveolar lavage and serum sample revealed the presence of CMV. Early diagnosis of cutaneous CMV and appropriate treatment are the key actions in management of patients with underlying immunodeficiencies to avoid further complications.

Survey of Mycoplasma pneumoniae in Iranian children with acute lower respiratory tract infections BJID

OBJECTIVES: Mycoplasma pneumoniae is an atypical pathogen, which is one of the major causes of lower respiratory tract infections (LRTIs) worldwide. This study was performed to determine the role of M. pneumoniae in acute LRTIs in children, who were referred to main pediatric hospitals in Shiraz, Iran, with the diagnosis of LRTI. Polymerase chain reaction method on a throat-swab specimen was utilized to detect M. pneumoniae. RESULTS: One hundred patients with acute LRTIs were investigated in this study. There were 10 positive PCR for M. pneumoniae (10%), including 6 of 62 hospitalized patients and 4 of 38 outpatients. All patients with LRTIs due to M. pneumoniae had cough.
